Patient-Reported Outcome Measures (PROMs)

Overview Humana PROMs

Cohere Health has developed a comprehensive Patient-Reported Outcome Measures (PROM) toolkit designed to assist providers in easy navigation of PROM tools per applicable body site. The PROM toolkit utilizes standardized, evidence-based tools endorsed by the American Academy of Orthopaedic Surgeons (AAOS) for the assessment of the patient’s functional disability.

Cohere uses patient-reported outcome measures as part of our clinical review process. Based on the diagnosis and services requested, you may be asked for a functional assessment score when answering the clinical assessment questions. PROMs are powerful tools to help providers understand the outcomes like pain and functioning that matter to patients. A few important things to note:

  • These questions are NOT required - if you do not have this information please enter "0".
  • If you use another type of functional assessment please include this information.
