Cohere Health has developed a comprehensive Patient-Reported Outcome Measures (PROM) toolkit designed to assist providers in easy navigation of PROM tools per applicable body site. The PROM toolkit utilizes standardized, evidence-based tools endorsed by the American Academy of Orthopaedic Surgeons (AAOS) for the assessment of the patient’s functional disability.
Cohere uses patient-reported outcome measures as part of our clinical review process. Based on the diagnosis and services requested, you may be asked for a functional assessment score when answering the clinical assessment questions. PROMs are powerful tools to help providers understand the outcomes like pain and functioning that matter to patients. Click on the tabs at the top of this article to view the specific tools for each payer.
A few important things to note:
- These questions are NOT required - if you do not have this information please enter "0".
- If you use another type of functional assessment please include this information.
The table below indicates which document should be completed by the patient for the specific body site and condition. To download and view the questionnaire, click the link within the table. If you are interested in the comprehensive toolkit, please click here.
